Insurance cards, X rays, MRI’s or scans, relevant medical reports, current medicine list

Insurance/Payment

Insurances accepted Medicare, BCBS, Medica, and Midlands Choice, (please ask if we are providers for your specific insurance prior to 1st visit)
Deductibles and Copays apply.

• We can provide an insurance claim form for out of network insurances or a receipt for HSA.
• Notify Staff Immediately if condition is due to a Car Accident or Work related injury (Workman’s Comp)

Not all treatments are covered by insurance. Payment due at time of service.

Non insurance covered services include Low Level Light Therapy, N.E.T, IonCleanse Footbath, and Nutritional Supplements

Payment forms accepted: Credit Card, Cash, Check ($30 return fee)
